Contractor Responsibilities

Must submit schedule forms 1 day prior to scheduled assessment time.
Contractors must provide a purchase order number on schedule forms for payment of no shows.

Participant Responsibilities

Must schedule at least 1 day prior to scheduled assessment time.
Must pay with credit card at time of scheduling assessment or come into office and prepay.
No Personal Checks.
Must arrive 15 minutes prior to start of skill assessment with picture identification.
No one will be allowed late entry - NO EXCEPTIONS.
